Sarah Hyland, famous for portraying Haley Dunphy in the ABC sitcom "Modern Family" has obtained a restraining order against her ex, Matthew Prokop and said he choked and threatened to kill her.

According to reports, a Los Angeles County judge put the restraining order in effect days before Matthew Prokop was to be released from a rehabilitation facility. Hyland, in her court records, claimed that the rehab center director was the one who warned her to "seek this order immediately due to Mr. Prokop's mental state and attitude" toward her.

The two have been in a relationship for four years, but the actress purported that Prokop has repeatedly abused her verbally and physically.

The restraining order, issued by Superior Court Judge Andrea C. Thompson on September 19 band Prokop from being within 100 years of Ms. Hyland. This includes his being prohibited from her home, workplace and vehicle.

Lee Sherman, the attorney of the Modern Family star, said that Hyland would not discuss the issue publicly since "the documents filed speak for themselves."

Prokop, who also worked with Hyland in Modern Family, was not available to comment on the issue. However, court documents suggested that he was not aware of the September 19 court hearing due to the fact that Hyland was afraid he would confront her prior to the hearing.

Hyland gave out a few details regarding the abuse. She said Prokop called her derogatory names like 'slut' and 'whore' on different occasions, and he would do so in public and in private.

The 5'2" actress also claimed that Prokop slammed her against a car door and attempted to strangle her after she begged him to stop calling her with vile names. The actress, who is suffering from kidney problems said she was not able to push him away because he outweighed her.

Hyland also said to have tried to terminate their relationship since she got scared for her safety. She even employed the help of her "Modern Family" co-stars to "intervene and help me peacefully end the relationship."

When she tried to break it off with him, Prokop allegedly "ran outside into the backyard and began screaming." He then told Julie Bowen, who plays Hyland's mother in "Modern Family" that he wished to make Sarah so miserable that she would shoot herself with a gun.

Prokop also made threats to burn down her house, and that she would never see her dog again unless she allowed him to take his belongings.

Hyland also said that her abusive ex-boyfriend threatened her through text messages and voice mails prior to going to rehab.
