Yorkville Endoscopy, the clinic that performed the throat procedure to host Joan Rivers, leading to her death, was found to have conducted many deficiencies, New York state regulators revealed.
The doctors at Manhattan's Yorkville Endoscopy clinic "failed to identify deteriorating vital signs and provide timely intervention during the procedure on Aug. 28," which triggered the death of Rivers due to the lack of oxygen supply to the brain, the New York Department of Health and Human Services has determined.

Then, the agency also noted many other misconduct, including failure to secure the patient's consent for the procedure, fault in administering the anesthesia Propofol, failing to take Rivers' weight, letting an unauthorized doctor perform the procedure and violating the patient's privacy by taking a cell phone photograph of the patient while the procedure was ongoing.
Rivers passed Sept. 4 at Mt. Sinai Hospital due to the complications at the conduct of the surgery.
Melissa River's spokesperson immediately issues a statement, which aired Rivers' disappointment of "the multiple failings on the part of medical personnel and the clinic," based on the reports by NYC regulators.
The statement also aired Rivers' outrage at the "misconduct and mismanagement now shown to have occurred before, during and after the procedure."
Furthermore, Rivers will do her best to "ensuring that what happened to her mother will not occur again with any other patient."
Joan Rivers' only daughter has already hired the service of a law firm to look into the possibility of suing the clinic.
Meanwhile, a statement from Yorkville Endoscopy emphasized that it has been "fully cooperative and collaborative with all regulatory and accreditation agencies."
With regards to the state regulators' findings on deficiencies, the clinic said that it has already "submitted and implemented a plan of correction that addressed all issues raised."
The regulatory agencies are already evaluating the clinic's corrective plan of action, the statement further revealed.
