The first ending is by touching the bonfire, where the player links the first fire. While this is fine, it does not give closure because the player was not really able to change the world and just kept the fire burning.

Another ending is welcoming a new world by having a Fire Keeper from the Firelink Shrine and finding an alternate location to Consumed King. At the top of the ladder which was dropped in the High Wall of Lothric, the player should go left and find a new place which will connect to the Consumed King. There is an illusory wall where the enemy Champion Gundyr will be fought. Beyond Gundyr, the player can find an item called Eyes of a Fire Keeper which can be presented in the other Firelink Shrine. By summoning the Fire Keeper at the Soul of Cinder bonfire, the player can break the fire cycle.

Another ending is a moment when the Fire Keeper communes with the fire and the player strikes it down. This is said to result in a shaky future.

The fourth ending is so complex it needs a complete guide found here.

Meanwhile, there are reports that the PS4 and PC game differ in frame rate performance. The PC is said to have a smooth 60 FPS run while the PS4 is limited at 30 FPS. It could be because of the 'Dark Souls 3' Patch 1.03 which was rolled out to fix FPS issues. It reportedly also boosts performance; especially with lower resolution at 720p.
